We understand that enrolling your dog or puppy in our Daycare or The Puppy Place for the first time can be confusing. Our Intake Form allows us to learn more about him or her, and must be reviewed by us before you can make a reservation for your dog’s first day.

We hope these instructions will be helpful.

Step 1: Request Services

From your Account Page or from the Drop-down menu

Step 2: Request an "Appointment"

This “appointment” is simply to bring you to the Intake Form. It is not an appointment or reservation for Daycare or the Puppy Place.

Step 3: Intake Form and Date

Select the Intake form in “Type”
Enter today’s date. This date allows our staff to review your form as soon as possible. It is not an “appointment.”

Step 4: Click "Book Now!"

Step 5: Select Your Dog's or Puppy's Name

Step 6: Fill Out the Intake Form

Make sure you enter any allergies, or “none” in that required field.

Step 7: Create "Appointment"

At the bottom of the form click “Create Appointment” button.

Step 7 sends the form to us for our review. You don’t need to do anything further until you hear from us with possible follow-up questions or instructions on how to proceed.
